Your primary care visit is designed to give you the right care at the right time. Knowing what you want to discuss helps you and your provider make the most of your appointment.

Think of your visit in three simple steps: before, during, and after.

Below, we’ll walk you through each step so you can feel prepared, confident, and supported every step of the way.

What do you want to get out of your appointment? Share this information when you call to schedule your appointment.

Instead of just saying, “I'd like a check-up,” mention the main reason for your visit such as questions about high blood pressure, medications, or a possible infection. Details help your care team plan better.

If your health, allergies, address, or insurance has changed, let your care team know before your next visit.

Keep your visit focused on your overall health, and ask only a few additional questions about your most important concerns.

Focus on the main reason for your visit so your provider can give it the attention it deserves. If you’re not sure whether another issue is related, ask your provider if it should be addressed today or scheduled for another visit.

 Your provider may not be able to cover everything in one appointment. Don’t hesitate to schedule a follow-up if you need more time.

To refill a prescription, request it from your provider or schedule an appointment if a visit is required.

Before leaving, repeat back what you need to do between visits and confirm the date of your next appointment.

Your After Visit Summary (AVS) shows your vital signs, test results, medications, and instructions from your visit. You can view it in MyChart or request a printed copy.

If something doesn’t match what you remember discussing during your visit, contact your provider ASAP.

Saban Community Clinic started in 1967 as the Los Angeles Free Clinic. Volunteers provided all services and private donations allowed it to remain free of charge. However, in 2012, we ceased being a “free” clinic in order to meet the growing needs of our community and became a Federally Qualified Health Center (FQHC). This change has allowed us to expand services and provide affordable, comprehensive medical, dental, behavioral health, and pharmacy services to our four locations. Plus, access to social services to a wider range of patients.

We accept various insurance plans and have a sliding fee scale based on income. We won’t turn anyone away, regardless of their ability to pay, and thanks to private donors and government funding, we subsidize millions of dollars in care for patients every year. While a lot has changed over the past 50 years, our founding belief remains the same: healthcare is a right, not a privilege. We serve all patients, regardless of their health coverage status or ability to pay. We offer many options to make quality care affordable.

As Required by law, Good Faith Estimates for patients who do not have insurance or who are not using insurance, will be provided. This includes an estimate of the bill for medical items and services. 

Is Saban a free clinic or do I need to pay for services?

Saban works to provide affordable quality healthcare regardless of immigration status, income, or race. We will ensure you get the care you need and provide options:

Call 323-653-1990 to schedule an appointment with a Patient Eligibility Specialist. They can help find health programs or enroll you into a health insurance plan (ie. Every Woman Counts, My Health LA).

In addition to enrolling in a health program or insurance, patients can also apply for the Sliding Fee Discount Program.

Patients can also explore payment plans if needed.

As Required by law, Good Faith Estimates for patients who do not have insurance or who are not using insurance will be provided. This includes an estimate of the bill for medical items and services. 

A Good Faith Estimate (GFE) is an explanation of expected costs report that is provided in advance of your appointment. It must include charge information for the primary service that Saban expects to provide, and for any items/services for which there is a separate charge that are “reasonably expected” to be provided “in conjunction with the primary service.” The estimate is based on information known at the time the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill. Make sure to keep a copy or picture of your good faith estimate.
