Your privacy rights

Patients’ health records contain personal health information, the confidentiality of which is protected under both state and federal law. We comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the Confidentiality of Medical Information Act (CMIA) to protect confidential healthcare information.


Your rights and responsibilities

At Saban Community Clinic, we want to ensure that each patient in our care is treated with dignity, respect, and compassion.

Likewise, the Clinic has the right to expect reasonable and responsible behavior from patients and those accompanying them (including service animals). The following is an outline of patient rights and responsibilities that serve as a foundation for a good relationship between our patients and the Clinic. We expect all individuals to demonstrate civil and respectful behavior while on our premises, virtual and/or digital communication, which include phone, text, or any other software/programs.
