If you are a current patient with a MyChart account, we recommend logging in to their system to request your medical records.

If you do not have a MyChart account or do not want to utilize MyChart, you can request your medical records by completing an online request through ShareCare. 

All patients have the right to request a copy of their personal medical records. The information contained in your medical records allows health care providers to determine your medical history and provide informed care. If you use MyChart, you can easily access your medical records without having to make a special request.

Requests are processed within 15 days after submission.

How long does it take to fulfill my request?

On your application, you can select to receive your medical records by MyChart, secure e-mail, mail, fax (less than 50 pages), or pick them up in person at our Beverly location. To maintain patient privacy we require picture identification before releasing your medical record copies.

If you are picking up your records in person, please call (323) 653-1990 (select option 2, and option 2 again), before arriving to ensure documents are ready. Pick-up is available Monday – Thursday 8:00 am and 4:00 pm, at our Beverly location:
