Covered services include:
- Exam (once every 12 months)

- Lenses (once every 12 months)
- Single vision
- Frames (once every 24 months)
- Contact lenses (once every 24 months, in lieu of glasses)

Employee Assistance Program (EAP)
Our employee assistance program is here to help you balance life’s difficult challenges. This confidential program is dedicated to supporting our employees and their families’ emotional health and well-being. The employee assistance program (EAP) is provided at NO COST to you through Magellan.

Flexible Spending Accounts (FSA)
The Igoe flexible spending accounts (FSAs) are one way to cut down on your expenses—and we offer two. With the Medical FSA, you set aside a portion of your pre-tax income to use towards eligible health expenses not covered by insurance such as dental or vision copays. Because that portion of your income is not taxed, you pay less in federal income, Social Security, and Medicare taxes. Dependent Care FSA allows you to set aside pre-tax dollars to use towards eligible child and adult care expenses. These dollars may be used towards daycare, before and after school care, and summer day camps. It’s the care your family needs, while you’re at work.
Saban Community Clinic is proud to offer robust retirement savings options through Voya Financial. We offer two Retirement Savings options:
- Traditional 403b – in this option contributions are taken pre-tax. You get a tax break up front and pay no taxes on the money you put in until you withdraw it.
- Roth 403(b) – in this option contributions are deducted after tax. The money you contribute has already been taxed. You pay no taxes when you make withdrawals after age 59 ½.
After 1 year of employment, Saban Community Clinic will match 50% of your contribution up to a maximum of 4% of earnings. In order to receive the maximum 4% match, you must contribute at least 8% of your earnings. These match contributions are subject to a vesting schedule of 20% per year until 5 years of employment when I will reach 100% vesting.
Life and AD&D Insurance
We want to ensure that the people who depend on you for financial support are taken care of. That’s why we offer life and accidental death and dismemberment (AD&D) to you and your eligible family members.
- Basic Life insurance provides your named beneficiary(ies) with a benefit of $50,000 in the event of your death and is provided at no cost to you through Anthem.
- Accidental Death and Dismemberment (AD&D) insurance provides specified benefits to you in the event of a covered accidental bodily injury that directly causes dismemberment (i.e., the loss of a hand, foot, or eye).
If you determine you need more than the company-paid Basic Life and AD&D coverage, you may purchase Supplemental Life insurance coverage through Anthem for yourself and your eligible family members.
SABAN Community Clinic understands that life happens. Our free long-term disability is here to help if you become sick or injured and can’t work (LTD) insurance plan has you covered by replacing part of your lost income. *
*The disability benefit will begin after 90 days and will be coordinated with any state disability payments.
Accident insurance can pay you money based on the injury and the treatment you receive, whether it’s a simple sprain or something more serious, like an injury from a car accident. Accident benefits pay in addition to other insurance and can be used to help cover gaps in health insurance or other expenses if the unexpected happens.
Critical Illness pays a lump sum directly to you at the first diagnosis of a covered condition. You decide how to spend it.
Hospital indemnity insurance pays a cash benefit if you or an insured dependent are confined in a hospital for a covered illness or injury.
Saban recognizes that pets are family too! This is why, we offer nose-to-tail coverage for a wide range of injuries, illnesses, genetic conditions, and emergency care for dogs and cats thru ASPCA Pet Health Insurance. Coverage is provided with no claim limits and offers unlimited benefits with an annual deductible.
Time off Benefits
We also offer a generous time off package for you to make work-life balance a reality. Our paid time off package includes up to 9 sick days per year, two (2) weeks of vacation, 13 SABAN Holidays, and 40 hours of CME hours for eligible employees. Vacation hours increase to three (3) weeks after 2 years of employment and four (4) weeks after 5 years of employment. Employees working over 20 hours per week receive pro-rated amounts based on full-time equivalency.

We prioritize career development by providing up to $1500 of Tuition Reimbursement to employees after one year of employment.
As a Federally Qualified Health Center, our employees may also be eligible for one or more of many loan forgiveness programs offered in the state of California.
