Saban Community Clinic is Los Angeles County’s medical home of choice for women, men and children in need of affordable health care specializing in preventive primary care, pediatrics, family planning, prenatal, dental and mental health services. Call us for an appointment at (323) 653-1990.

The following forms of identification will help us determine which health care coverage you may be eligible for. Please bring to your next appointment:

  • Proof of income (paystub, unemployment check, letter from Social Security, tax return)
  • Proof of address in Los Angeles County (utility bill, lease agreement, California driver's license or ID). If you are currently homeless, we will assist you.
  • Photo ID (California driver's license or ID, other Photo ID)
  • Proof of citizenship (birth certificate, green card, passport, certificate of naturalization). If you have no proof of citizenship, we will still assist you.

You will not be denied services based on your income, housing or citizenship status.

Paying for health care: Sliding-Fee Scale

Paying for health care services at Saban Community Clinic is based on a sliding-fee scale, which takes into account a patient's family size and income in accordance with federal poverty guidelines. Many patients qualify for health care coverage or insurance that pays for the services they receive. Our Enrollment Specialists will help you participate in the optimal health care coverage for which you are eligible.

Saban Community Clinic serves all patients regardless of their insurance status.
We accept most public health insurance plans and provide assistance for insurance enrollment.

Discounts are offered to self-insured patients depending upon family size and income.

Health care for all

National health care reform has increased grant dollars to Federally designated Community Health Centers. In 2012, Saban Community Clinic was designated a Community Health Center under PHS Section 330, thereby qualifying our clinic for increased government funding. Greater financial sustainability guarantees that the Clinic will continue to be recognized as a leader in the health care safety-net, and offers new opportunities to access federal funding, technical assistance and other resources that are critical as we implement health care reform. It is important to note that though some clinic policies have changed, we remain true to our mission of providing quality care to those who need it regardless of ability to pay.
