Performance Measurement and Quality Improvement

At Saban Community Clinic, we value continuous service quality improvement to make the experience of our patients and families the best! As we work to become a Patient Centered Medical Home (PCMH), an integrated approach to care focused on patient experience, we are tracking quality improvement performance to monitor how we are delivering overall patient care to enhance quality and determine improvement areas.

A summary report of the Clinic’s quality performance measures tracked through the Third Quarter of 2016 can be viewed by clicking here. The report shows individual clinician percentages of patients assigned to each provider for the current year. Patients must have had a visit during 2016.
